Processing Acknowledgment Form

The United States Postal Service® (USPS®) requires that each NCOALink™ licensee have a completed NCOALink Processing Acknowledgment Form for each customer.

This form should be completed by an officer of the company and a new form is required every year.

When completing this form the USPS requires that you provide a full 6 digit NAICS code. This code describes the industry that your company is in. If you do not know your NAICS code, you may look it up here.

We recommended that you fill out this form within Acrobat before printing it out. This will insure the legibility of the document. After completing the form it will need to be printed, signed and returned to Peachtree Data via fax.
